Your Insurance Form
If you have dental insurance, please take the time to complete the following paperwork and bring it along to your first appointment. This will allow us to directly bill your insurance provider, simplifying your appointments and facilitating each procedure at our office.
New Patient Form
Please take the time to complete this form, which will provide us with all the details necessary to supply you with a personalized treatment program. Bring this with you to your first appointment, we will review your responses to assess you current and past dental health for your ongoing dental care at Okanagan Smiles!
Child’s Health Form
For children up to the age of 12, we require a health history form to be completed by their parent or guardian prior to their first appointment. We would like to develop a treatment plan that is specific to your child’s special case, and this will allow us to shape their smile for decades to come!
Credit Card Agreement
This will allow us to directly bill you through your credit card.
Financial Policy
